Facility Use Request Form

Proof of insurance and naming Prayer and Word Church (PAW) as an additional insured may be required.

Liability Waiver and Agreement

I acknowledge that I am requesting use of the facilities of Prayer and Word Church and agree to assume full responsibility for the use of the premises during the approved time period.

I agree that I, and any organization or group I represent, will be responsible for the conduct and safety of all attendees, participants, guests, and invitees associated with this event. I further agree to use the facility in a safe and lawful manner and in accordance with all applicable laws and the church’s Facility Use Policy, and to ensure that all attendees, participants, guests, and invitees comply with the same, including adherence to all space-specific rules such as no food or drink in the sanctuary.

I understand and agree that Prayer and Word Church, its officers, members, and representatives shall not be held liable for any injury, loss, damage, or theft of personal property that may occur in connection with the use of the facilities.  I further acknowledge and agree that I, and any organization or group I represent, as well as all attendees, guests, and invitees, assume all risk associated with the use of the premises, including parking areas and surrounding grounds, which are used at their own risk.

I agree to be financially responsible for any damage to the church property, equipment, or grounds caused by myself or any attendees, participants, guests, or invitees associated with the event, and I understand that such costs may be deducted from any deposit or billed to me directly.

To the fullest extent permitted by law, I agree to indemnify, defend, and hold harmless Prayer and Word Church, its officers, members, and representatives from and against any and all claims, liabilities, damages, or expenses arising out of or related to the use of the facilities.

I acknowledge that submission of this request does not guarantee approval and that the church reserves the right to approve, deny, or revoke facility use at its discretion.
